Hi - My heart goes out to you and your son. Having IBS as a child is just awful, and more difficult than dealing with it as an adult.
Can you at least get your son to start taking a soluble fiber supplement every day? There are powders or pills, whichever he prefers. Here's info on that web page. This really should make a huge difference for him, help normalize his bowel function, and stop the pain.
Can you get him to follow this info on breaking the cycle? web page If he can just get stabilized it may be worth it for him to eat some foods that aren't his favorites. Then he can carefully expand his diet.
Pepogest should not cause heartburn - the capsules don't dissolve until they reach the intestines. Also, he can have room temperature or cool herbal ice teas - they don't have to be hot. Try fennel (tastes like licorice) or ginger, and sweeten with a little white sugar if he likes.
Some kid friendly recipes that will help stabilize him:
honey snack mix zucchini bread banana bread french toast peppermint fudge cake rice pudding

I really hope he's feeling better soon.
