You definitely don't soak them every time (what a hassle that would be!). They'll stay all puffed up for quite a few rounds in the microwave - I use mine mostly once a month for cramps, probably heating the things 3-5 times a day, and I've only rehydrated them a handful of times in years.

If you get the pack really hot (sounds like you did) it should stay hot for a couple of hours. It will gradually cool down and be warm for another few hours. It helps to keep it wrapped in a towel or under a blanket. I gradually move it from outside the the towel, to outside my pajama pants (confession - I don't get dressed until I leave the house, so I'm usually typing away in pj's), to inside my pants against bare skin.
