Fructose test

Ok, so I went for my fructose breath test yesterday. I have a good bit of fructose in my diet and thought I could handle it just fine...but in the process of ruling everything out, I really figured I would just do it. Well, I drank the solution, my numbers rose right away, but not enough to be positive yet. After the first round, I got awful cramping, lots of diarrhea and was so sick! It was miserable. I was surprised then when the rest of the breath test came back normal...my numbers went right back down. What the heck?! The nurse even said it was pretty bizarre. She said she normally sees it continue to rise, not drop down. And she said that I certainly had the right reaction for a fructose problem..
Can anyone give me some insight? I'm waiting to hear if the doctor can give me some impression... I'm afraid they'll just say, oh well. It's negative. But such a weird reaction!
However, I did get another recommendation for an ibs specialist at Cleveland clinic, so I will see him in April. Hopefully this doc will a plan. I'm so sick of this!

If you had that sort of reaction it is probably best to reduce or eliminate foods with excess fructose for a while and see how it goes. The breath test is not particularly sensitive. Best listen to your body.

SO sorry you had to get sick like that.

I don't know if this will be part of the picture for you, but have they done the test for methane? When I went for my test they said the same fasting applied to both tests (hydrogen and methane), but they do run the tests differently (I think I drank a different solution, but not sure). It might be worth asking if you're not sure. My hydrogen levels rose, but only slightly, and were in normal range. My methane levels were ridiculously high. You can definitely feel very sick from either one!! I had to run for the bathroom less than 10 minutes in to the test (and I'm an IBS-C!), and I was full of gas when my GI listened to my stomach.

I hope your next appt is helpful and that you get some good answers.

if you had a bad reaction, avoid fructose and see if it helps. It can't hurt to try and eliminate it.

Glucose not fructose is usually used for methane test. They use to use lactulose but it gave many false positive tests so it has been switched to glucose. There should not have been a negative reaction to the glucose solution other than an increase in methane. It is quite possible to have a negative reaction to the fructose.

It is fairly simple. There are 5 FODMAP groups. Fructose is one of them. Through a process of elimination you work out which of the other four groups give you problems.

So I'm starting the diet and trying to figure out if there is a way to sweeten my tea? If table sugar is a no and honey is also...are there any ibs friendly sweeteners?

Sugar is not forbidden on the FODMAP diet. White/brown sugar (sucrose) contain exactly equal amounts of glucose and fructose whereas honey contains more fructose than glucose. It is foods that contain more fructose than glucose that are a problem. This is described in the first link in my signature where it says

"Fructose-containing foods with a 1:1 ratio of fructose to glucose are generally well tolerated on the FODMAPs diet. Conversely, foods with excess fructose compared with glucose, such as apples, pears, and mangoes, will likely trigger abdominal symptoms."

White/brown sugar has a 1:1 ratio of glucose to fructose and shouldn't be a problem. I am very sensitive to foods with excess fructose but white/brown sugar don't bother me at all. Also, you can use Stevia but make sure it doesn't contain IBS unfriendly additives such as inulin (chicory root).
