My coworkers want to eat out all the time

Need advice FAST (have to handle this situation tomorrow!).
My coworkers LOVE to eat lunch out together. I'm not fond of the concept, partly because my blood sugar dictates that I eat small snacks periodically through the day (not one big lunch at one sitting) and my IBS makes it a lot easier for me to bring my lunch, containing what I know I am comfortable eating.
What's the best way to handle frequent "team lunches" (where you kind of HAVE to go, or look like a jerk), in restaurants? I want to just carry in my food, but I never know how the restaurant will deal with that and I don't want to embarrass my team. On the other hand -- I really just want them to stop inviting me, and understand that I cannot physically handle these lunches.
What's the most tactful but effective way?

Hi Cara - this is a situation were brutal honesty might be the best policy. I'd approach the team leader and tell them the truth - you have a medical condition that makes it extremely difficult to eat at restaurants.
If there is a local restaurant where you could find safe meals, you might ask that they make that the designated lunch spot.
If you know the restaurant they're going to, you could call them ask their policy for bringing in your own food. I'd explain you're with a group, and everyone will be ordering, but you have a medical condition that forces you to bring your own food. If you've spoken to an owner or manager in advance, you might be able to bring your own lunch without problem.
There might also be a nice place where you could all eat lunch together (a park, etc.), but they bring food to go from the restaurant they like and you bring your own.
