We're both busy most of the time, but we try to do little things. I don't think we've ever bought each other flowers or anything... but we write each other cute notes sometimes, or emails, or pick up a little something the other person would like if we're out shopping or something and surprise them with it... no grand gestures here, but it's the little things that are nice  We definitely did more stuff like that earlier in our relationship, but 3 years in now, we still try to do some things. (We don't need quite the same amount of mutual assurance now though I think, hehe.)
On the other hand, my parents, who have been married for 25+ years surprise each other with stuff all the time. Always buying each other gifts and flowers just because!
