Good for you! I do the same thing, writing things down, and sometime even reading off the page, lest I forget something, LOL! It really pays off big to be proactive about your health, and I can tell you're a smart cookie and get it in a big way. I know it's easy to get derailed, especially when stress and life get in the way, so I hope you don't get discouraged. I lean on the boards here a lot when that happens, and it's very comforting to know that every here knows what I'm going through, because they've been through it themselves!!
