For example, has she had a colonoscopy to rule out IBD? Stool samples for parasites? Blood work for infections or thyroid problems? Gynecological exams? I think it's less common for people to develop IBS in middle-age, but has she recently had a GI infection or is she taking medications that may possibly be triggering the D?
