Say listen, I have a great fear of this type of surgerya nd without it I am liable to be harmed when you try and give me the needle because I WILL twitch and squirm. Just say that is a fact. You can also just say you are under the care of counselor or what not and that you get panic attacks from this type of stress. That should do the trick.
