The pectin in apples don't affect the way one reacts to fructose. The only determining factor is the amount of excess fructose (i.e. how much more fructose than glucose) a food contains. Apples contain about 3 grams of excess fructose per 100 grams.

Loose stools are not automatically suggestive of a risk of malnutrition. Watery stools passed many times a day would be a concern.
