Dry Rice/Pasta to Cooked Pasta Conversion

It's really annoying how most companies post on their labels nutritional facts for rice/pasta as dry.
Like who the heck eats rice/pasta dry?
Most of the time, most people will just cook the whole package, and save the rest for leftovers, because it's convenient as opposed to measuring out 4oz and cooking 4X per day.

Many of us need to monitor the amount of calories/carbs we eat per meal as well...and this makes it quite difficult.

So I'd like to ask

1) Generally speaking X oz of dry rice=X oz of cooked rice
2) X oz of dry pasta=X oz of cooked pasta

What I do is cook, then weight, and it makes it much harder to get an accurate summary of my intake.
It's completely inconvenient and out of the question to measure beforehand because many times I'm not the one cooking/the food is already prepared and quite simply it defies common logic.
