First off, I'm sorry that you're feeling so sick. I hope you are under the care of a good doctor, especially with the significant weight loss.
I have some thoughts on things you said: 1) I wonder if this sense of fullness you have is true fullness, or if it gas or bloating in the stomach? I often feel extraordinarily full, even when I know it's not possible based on how much I've eaten and how long it's been since I ate. Have you tried fennel tea for this feeling of fullness? It works wonders on gas, and may ease that symptom a little.
2) If your stomach has shrunk, then I would focus on eating a tiny bit very frequently. Like maybe half a piece of toast with 4 ounces of juice, then an hour or two later, eat the same amount. So you're eating every hour to two hours.
3) A great food for you might be smoothies. You can pack a lot of nutrients in using IBS friendly fruits, juices, pasteurized egg whites or soy protein powder, even oats for some complex carbs. Again, because you can't eat much in one sitting, try drinking a small amount of a smoothie every hour or so.
