It's definitely possible that your IBS symptoms change over time. I was always a straight D... now I'm an A.
However, you still don't sound like a D to me. D is diarrhea, and believe me, you know darn well if you have IBS-D! LOL!! Pain is a completely different issue from D, C, or anything else.
I have to admit that I don't know how to best deal with the pain, because I (thankfully!) never had much of it. However, the best things I've found for spasms (which, I'm assuming, are what's causing your pain) are:
1. Peppermint - tea works, capsules are stronger 2. A good heating pad for the really bad days 3. Following the IBS diet, making sure to get plenty of soluble fiber
If you're already doing that, you might want to talk to your doctor about the possibility of going on an anti-spasmodic drug. But, honestly, I found that peppermint worked as a better anti-spasmodic than Bentyl, so you might want to just try that first.
