We never ask for your name and all the other information (such as age, gender, etc.) is purely optional, although it may help researchers analyze geographic/demographic trends with IBS/FBD patients. The company that designed the survey has a link on the main page, they are a leading biomedical research company, focused on the gut (to put it plainly).
