I am sorry you are hurting so much. I know in Mi getting disability is not an easy task. I have moderate to severe ra (rheumatoid arthritis) and because I am only 36, have been told it would be basically impossible to get disability. I would basically have to declare bankruptcy before getting disability.
It sucks to be in pain and still have to make a living. You have been around here long enough to know all the common answers of diet, exercise, blah, blah,. blah. Have you tried switching doctors? Have you tried the newer antidepressants that also help with pain, like cymbalta? I would assume that you have tried the anticolonergics by now, like Levsin and bentyl? I am just throwing out ideas here....exactly what are your symptoms still?
Love and hugs
